Your signature line is required so Staff have adequate information to respond accurately to your progress report.
-Click the green 'My Account' button at the top right of each page.
-Click the green 'Profile' tab in the middle of the page. (This button is hiding just below 'Watched Topics')
-Type the info in the space provided at the bottom of this 'edit profile' page. (make a copy just in case it's lost in cyberspace)
-Abbreviate when possible. There is a limit of 255 characters. Limit your signature to 2-3 lines. (This will minimize the extra space that signatures occupy on the site)
-Click 'Save' on the bottom of the page.
-Check one of your posts to see that all information has been saved.
Your signature will appear at the bottom of each of your posts when you click 'reply' even though it does not appear in the reply window when you are typing a message.
Whenever you update your signature line, it will change in all of your posts, both past and future.
The signature line is a tool for moderators to use to help monitor your progress......please include the information listed only.....do not include information not listed. Moderators may edit your signature line to exclude irrelevant information or add pertinent data. If you note an error, please PM a moderator.
Please include the following info in your signature line in this order:
-Diagnosis (or key symptoms if no diagnosis)
-Initial 1,25D (optional)
-Start date of the phase one; e.g. Ph1Mar07
-start date of modified phase 2; e.g. ModPh2Jun07
-Start date of phase 2; e.g. Ph2Dec07
-Start date of phase 3; e.g. ph3 6/11/07
-Latest 25D and date (required) e.g. 25D12 (Dec07)
-Medications you take every day; e.g. thyroid supplement, Lasix (do not include doses)
-Measures regarding natural light; e.g. NoIRs, covered up, low lux home, light exp. r/t work or commute or child care (do not include sunscreens)
We will assume you are not eating foods high in vitamin D. "covered up" means all exposed skin except face. No need to include these details.

Do not include:
-Your email address. (include it in your profile)
-Links to other websites. (you may include them in your profile)
-names of MP meds (report these each week in your report)
-info unrelated to the MP
