I'm having difficulty getting clear about the difference between "genuine" vitamin D deficiency and a high 25D/1,25D ratio resulting from Th1 disease.
I read "When patients are Vitamin D deficient, the parathyroid hormone increases and drives the renal 1-alpha-hydroxylase,so that 1,25(OH) vitamin D levels increase. Only in severe deficiency, when substrate is depleted, does 1,25 (OH) vitamin D become low. Partially treated vitamin D deficiency also results in marked elevations of 1,25(OH) vitamin D levels"
I understand that Th1 disease cases rapid conversion of 25D to the metabolite, 1,25D, so this could result in low 25D and high 1,25 D. But how are these people distinguished from those described above - with partially treated D deficiency,whose 25D is low, with high 1,25D? And what about those whose levels are both low? Of course this could be the mishandling of the sample, lab error, etc., but let us assume it is an accurate result. Does this indicate "true" vitamin D deficiency?
My question arises from recent test results after 10 months break from the MP and no avoidance of D foods or light.
My 25D is <18nmo/L (the lab's detection level)
1,25D is 50pmo/L
During my two years on the MP my D levels were checked monthly and both have been consistently low, despite using different labs, but never as low as this. My ionized calcium is also very low.
How do I know whether I have a "real" vitamin D deficiency, or whether this result indicated worsening Th1 disease?
I"m sure I"m missing or misunderstanding a basic concept - please put me right!

There is no such thing as "Vitamin D Deficiency." The body makes all the vitamin D it needs. It is made from 7-dehydrocholesterol. UVB light is not necessary at any point in this process.
The concept of D-ratio is one I no longer use. After we made the breakthroughs (late in 2005) we were able to define exactly how the D metabolism works. The slide showing the complete metabolite control system is in my Bio21 and AAEM presentations on the "Science" DVD.

Thank you. I need to keep reading I see. Is there no such thing then as Ricketts caused by D deficiency? Is there no danger to bones if D levels become very,very,very, low? Would D food and light avoidance still be recommended on the MP?

At Trevor's patient meeting here in Adelaide he mentioned that Vitamin D is, in fact, osteoclastic (sorry if I've spelt it wrong). Osteoclastic = breaks down bones. Not only is Vit D not needed to build bones, but it is actually detrimental to bones.
